Accommodation Requests

This form is for all NEW students to Student Disability Services in the Access and Success Office. After completing this form, the final step is to schedule an appointment in Starfish to discuss reasonable accommodations and put accommodations in place. 

  This form is for all RETURNING students to Student Disability Services in the Access and Success Office that have received Accommodations at ÁùºÏ±¦¿â and would like to renew their current accommodations with NO changes. You are no longer REQUIRED to meet with and Access and Success staff member to renew the same accommodations, but if you choose to meet with us, please schedule an appointment in Starfish with Michelle Laughlin.

This form is for all RETURNING students to Student Disability Services in the Access and Success Office that have received Accommodations at ÁùºÏ±¦¿â and would like to modify or CHANGE their current accommodations. Depending on the change, an Access and Success staff member may be in touch to schedule a meeting to discuss more. You may also schedule a meeting in Starfish if you would like with Michelle Laughlin.
